I have installed the short form module and set it as per the README.txt documentation. This created a block for my form under admin/build/block but no matter what I do this block is not remembering any changes I make to it; when I first "configure" it, none of the radio buttons are preselected, as would be usual for a new block, etc
The end result is that it is actually impossible for me to turn this block on by assigning it to a region.
